An event regarding revision surgery involving a triathlon insert was reported. The event was not confirmed.Method & results: -device evaluation and results: not performed as product was not returned.-clinician review: a review of medical records indicated "root cause analysis: the cause of this event cannot be determined with certainty with the information provided. The reason stated for revision of the primary implant was due to "status post left tkr with prosthetic joint implant failure.".This is a nebulous and imprecise diagnosis. The term "implant failure" is often used to describe simple failure of a total knee arthroplasty due to myriad causes including infection, instability, loosening, impingement, etc.In most cases this does not relate to actual mechanical failure of the implant such as breakage. Therefore it is impossible to assign any causality to the implant with this imprecise diagnosis." -device history review: all devices were manufactured and accepted into final stock with no relevant reported discrepancies.-complaint history review: there have been no other similar events for the reported lot. Conclusion: a review of medical records indicated "root cause analysis: the cause of this event cannot be determined with certainty with the information provided. Therefore it is impossible to assign any causality to the implant with this imprecise diagnosis." the exact cause of the event could not be determined because insufficient information was provided.Additional information including operative reports, progress notes, x-rays and return of the device are needed to fully investigate the event. If further information becomes available or the product is returned, this investigation will be re-opened.
