Event or Problem Description
The customer reported via phone call that the reservoir was not able to locked in place.The customer¿s blood glucose was 125 mg/dl at the time of incident.Customer stated the threading around battery chamber was cracked and they could not get the battery to stay in.The customer was advised that the insulin pump needed to be replaced and was advised to discontinue use of the insulin pump and to revert to the backup plan.The insulin pump will be returned for analysis.

A test reservoir was installed and did not lock in place due to completely broken reservoir tube lip.Unable to perform the displacement test due to completely broken reservoir tube lip.Device had missing reservoir tube o-ring, broken battery tube threads.
