Event or Problem Description
Attorney alleges that the patient underwent surgery for implant of an unspecified bard/davol perfix plug on (b)(6) 2011.As reported, the patient is making a claim for an adverse patient outcome against the bard/davol perfix plug.It is alleged that the patient had subsequent surgical intervention due to the hernia mesh device.As reported, the attorney alleges patient experienced emotional distress and the device was defective.

Event or Problem Description
Attorney alleges that the patient underwent surgery for implant of an unspecified bard/davol perfix plug on (b)(6) 2011.As reported, the patient is making a claim for an adverse patient outcome against the bard/davol perfix plug.It is alleged that the patient had subsequent surgical intervention due to the hernia mesh device.As reported, the attorney alleges patient experienced emotional distress and the device was defective.Addendum per additional information provided: (b)(6) 2011 - patient was diagnosed with indirect left inguinal hernia thereby underwent open repair with implant of perfix plug (device #1).Per operative notes, ¿a small perfix plug was placed into the defect and sutured it to the transversalis fascia, shelving edge of the inguinal ligament, tendon of the transverse abdominus muscle and the internal oblique and transverse abdominus muscle superiorly and placed a patch over the posterior wall of the inguinal canal and sutured.¿ (b)(6) 2012 - patient had left groin pain thereby underwent open repair with the partial removal of mesh.Per operative notes, ¿the mesh plug was dissected free, and another small unknown mesh plug and patch was placed into the defect and sutured.¿ (note: the plug and patch implanted in this procedure was unknown) (b)(6) 2012 - patient was diagnosed with left groin pain thereby underwent open repair with the removal of mesh.Per operative notes, ¿a small portion of external oblique fascia was excised and dissected out most of the edges of the mesh and removed the prolene sutures.Freed up the scar from the mesh.¿.
