Event or Problem Description
The customer reported via phone call that their insulin pump had reservoir not able to locked in place.The customer¿s blood glucose was 135 mg/dl at the time of incident.Customer reported that the insulin pump cracked at top and bottom half of the reservoir chamber.The customer was advised that the insulin pump needed to be replaced and was advised to discontinue use of the insulin pump and to revert to the backup plan.The insulin pump will be returned for analysis.
