Correction: lot code updated an event regarding a dislocation involving an all poly constrained liner was reported. The event was confirmed.Method & results: device evaluation and results: visual inspection was performed as part of the material analysis report (mar), dated 31-october-2019.This inspection indicated: nothing noteworthy was observed on the surfaces of the locking wire of the insert.Damage was observed on the uhmwpe acetabular insert consistent with the explantation process.A material analysis has been performed. The report concluded: damage consistent with the implantation/explantation process was observed on the stem (neck and main body), articulating surface of the bipolar outer femoral head, distal rim of the femoral head, and acetabular insert.Based on the given information, no identifiable material or manufacturing discrepancies were observed on the surfaces examined.Clinician review: a review of the provided medical records and/or x-rays by a clinical consultant indicated: a review of the provided medical records by a clinical consultant stated the following comment: one undated x-ray and intraop photos confirms disassociation of constrained liner, need additional information; primary and revision operative reports, clinical and past medical history, additional serial dated x-rays and examination of explanted components.Device history review: could not be performed as lot code information was not provided.Complaint history review: could not be performed as lot code information was not provided. Conclusion the exact cause of the event could not be determined because insufficient information was provided.Additional information including operative reports and progress notes are needed to fully investigate the event. If further information becomes available this investigation will be re-opened.
