Additional Manufacturer Narrative

One dragonfly duo imaging catheter was received for evaluation.Visual inspection revealed the catheter sheath had been separated at the distal tip and was not returned with the device.Due to the received condition of the device, the catheter damage could not be conclusively attributed to the reported event. The device history record was reviewed to ensure that each manufacturing and inspection operation was performed. The results of the investigation concluded that the catheter distal tip was separated from the sheath, distal to the lens, and was not returned with the catheter. The cause of the separated distal section is consistent with damage during use.
