Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h6: a product investigation was conducted.Visual inspection: the periarticular aiming arm for 4.5mm lcp prox tibia pl-lt (p/n 03.120.004 lot 3229797) was received showing slight wear across the instrument from consistent use and reprocessing over the part¿s lifetime. The following mating guide sleeves were returned as a concomitant device without an alleged complaint condition.Product code: 03.120.014, lot #: 3107680, qty: 2.Upon visual inspection, there is no evidence that these devices contributed to the complaint condition, no defects/deformities were observed on the mating spring tabs of the guide sleeves, and therefore no additional investigation will be performed on these devices.Functional inspection: functional testing was performed with the returned mating concomitant guide sleeves by assembling the guide sleeves into each hole of the periarticular aiming arm for 4.5mm lcp prox tibia pl-lt (p/n 03.120.004 lot 3229797).Both guide sleeves were not tightly secured when assembled onto the aiming arm, there was toggling of the guide sleeves in each hole. The toggling of the guide sleeves from the aiming arm can contribute to the complaint condition of misalignment.Dimensional inspection: measured dimensions: all aiming hole diameters measured approximately 16.17-16.20 mm, towards the upper limit of the specification result: all holes are conforming to the specification, however, the likely reason for the toggling is due to a combination of wear/consistent use of the self-retaining slots/holes/features of the aiming arm, possibly warping/deforming the features. No further dimensional inspection of other self-retaining/mating features of the aiming arm will be conducted.Document/specification review: during the investigation no product design issues or discrepancies were observed that may have contributed to the complaint condition. Conclusion: the complaint condition is confirmed for the periarticular aiming arm for 4.5mm lcp prox tibia pl-lt (p/n 03.120.004 lot 3229797) as the toggling of the guide sleeves from the aiming arm can contribute to the complaint condition of misalignment. The self-retaining slots/holes/features of the aiming arm are likely warped/deformed from consistent use over the part¿s lifetime.During this investigation no product design or manufacturing issues were observed that may have contributed to the complaint condition.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post market safety surveillance activities.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.H3, h4, h6: a device history record (dhr) review was conducted: product code: 03.120.004, lot #: 3229797, manufacturing site: hägendorf, release to warehouse date: 28.Aug.2009.A manufacturing record evaluation was performed for the finished device lot number, and no non-conformances were identified.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
