Additional Manufacturer Narrative
|
|
The subject device was returned and visually evaluated. It was confirmed that the screw had not been fully secured in the handle and came loose as was reported.Thread tap markings were missing from the handle where the screw was inserted.Based on our evaluation we have identified the most likely root cause to be manufacturing related. To date, this is the first complaint for this production lot of (b)(4) units released for distribution in july of 2017.A review of complaint history finds this to be the only report of a misassembled screw for the permafix fixation devices.
