Event or Problem Description
As reported, during peripheral intervention of the arteries in the leg, a flexor raabe guiding sheath separated into three pieces upon removal of the device. The device was removed with the aid of a guidewire and clamps.A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.

Event or Problem Description
Additional information was received (b)(6) 2019 regarding the patient and device event. As reported, access was gained via the right common femoral artery to contralaterally treat the superficial femoral artery. The patient's anatomy was calcified.A cook amplatz 260 wire guide was utilized in the device, and the device was used alongside a cook 5fr straight catheter and other manufacturers' stent and balloon.5000 iu intravenous heparin was also administered. After having been in place for less than an hour, separation occurred at the shaft of the device during removal, which was met with resistance. The dilator was not in place when separation occurred, and the sheath was not withdrawn with the dilator in place. After the device was removed, no tip damage was observed, but sharp edges were observed to the shaft of the device. The patient's outcome was reportedly "good.".

Additional Manufacturer Narrative
Investigation ¿ evaluation: reviews of the complaint history, device history record, documentation, drawing, instructions for use (ifu), quality control procedures, specifications, and a visual inspection of the returned device were conducted during the investigation. The visual inspection of the returned package confirmed that one flexor raabe guiding sheath was returned used and damaged for investigation. The sheath was returned in two sections.Material separation as noted at the hub.The flare and some sheath material were still between the cap and check-flo.The separated tubing was severely elongated and measured 74cm.There was 2.5cm of coiling exiting the proximal section of the sheath. The distal tip of the sheath was not damaged.Additionally, a document-based investigation evaluation was performed. There is no evidence to suggest the product was not made to specifications.A review of the device history record showed no nonconforming events which could contribute to this failure mode.It should be noted there were no other reported complaints for this lot number.Furthermore, reviews of the manufacturer¿s instructions, drawings, specifications, and quality control procedures were conducted, and no gaps were discovered. An ifu is also provided with the device, which states that reinsertion of the dilator prior to removal of the sheath ¿increases the strength of the sheath and lessens the risk of device separation¿ and suggests insertion of the dilator prior to removal if resistance is encountered or anticipated during withdrawal of the device.Based on the information provided and the examination of the returned product, investigation has concluded that this event can be traced to patient condition and unintended user error.Reportedly, the patient¿s anatomy was calcified, and the dilator was not reinserted prior to the removal attempt as per the ifu.We will continue our monitoring of similar complaints and have notified the appropriate personnel of this event.Per the quality engineering risk assessment no further action is required. This report is required by the fda under 21 cfr part 803.This report is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement made in it is intended to be an admission that any cook device is defective or malfunctioned; that a death or serious injury occurred; or that any cook device caused or contributed to; or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
