'it is alleged that "[pt] received an unknown cook filter in 2009'.It is alleged that [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.
 
Event Description
Patient allegedly received an implant in 2009 due to post pulmonary embolism.In addition, the patient received a previous greenfield ivc filter in(b)(6) 1983 at another facility for "sepsis caused by appendix ruptured caused pulmonary embolism in lung".Patient is alleging vena cava perforation, organ perforation, unable to be retrieved, bleeding and tilt.The patient further alleges, "1) continued dvt's with device, bleeding episodes due to rupture/broken pieces travelled to lungs and currently body is treating as foreign object an contributes to ongoing deep vein thromboses (dvt); 2) inability for exercise, travel restrictions affecting my employment; 3) medications (lovenox, eliquis) at above prescribed dosages; 4) pain in abdominal area; 5) walking restrictions; 6) hypertension." the patient further notes limitations in "walking, standing or sitting long periods of time, driving not to exceed 2 hour increments, running and sleeping due to pain".Per the nuclear medicine pt/ct skull base to mid thigh dated (b)(6) 2018, impressions: some of the filter struts have penetrated through the wall of the ivc into the pericaval/mesenteric fat.The filter is tilted posteriorly with its cone penetrating through the wall of the ivc into the perivaval fat.This may render the filter non removable.

Device code(s): appropriate term/code not available (3191) was selected for the alleged device perforation and tilt.Investigation: investigation is reopened due to additional information provided.The following allegations have been investigated: fracture, vc/organ perforation, migration, dvt, unable to be retrieved, tilt, foreign objects remain in patient, bleeding, use of lovenox and eliquis above prescribed doses, pain in abdominal area, hypertension, sleeping limitations due to pain and physical limitations.The reported allegations have been further investigated based on the information provided to date.Filter fracture has been reported and may be either symptomatic or asymptomatic.Fracture of a filter leg may be due to repetitive motion on a filter leg in an unusual, stressed position, such as a filter leg penetrating/perforating the ivc; or a filter leg being caught in a side branch (e.G., a renal vein).Other potential causes of filter fracture may include excessive force or manipulations near an implanted filter (e.G., a surgical or endovascular procedure in the vicinity of a filter).Retrieval of a fractured filter or filter fragments (including embolized fragments) using endovascular techniques has been reported.Potential adverse events that may occur include, but are not limited to, the following: filter fracture, filter or filter fragment embolization, trauma to adjacent structures.Filter interacts with ivc wall, e.G.Penetration/perforation/embedment.This may be either symptomatic or asymptomatic.Potential causes may include improper deployment; and (or) excessive force or manipulations near an in-situ filter (e.G., a surgical or endovascular procedure in the vicinity of a filter).Potential adverse events that may occur include, but are not limited to, the following: trauma to adjacent structures, vascular trauma, vena cava perforation, vena cava penetration.Ivc occlusion/ thrombosis, new dvt, ivc stenosis as a reported complication, is a known risk in relation to filter implant and is well documented in the clinical literature and in clinical practice guidelines.This is supported by the clinical evidence report established to assess available clinical data to identify and evaluate the clinical safety and performance of the cook vena cava filters.Potential adverse events that may occur include, but are not limited to, the following: vena cava occlusion or thrombosis, vena cava stenosis, deep vein thrombosis.Physician practice guidelines and published guidance from regulatory agencies recommend that patients with indwelling filters undergo routine follow-up.The risks/benefits of filter retrieval should be considered for each patient during follow-up.Once protection from pe is no longer necessary, filter retrieval should be considered.Filter retrieval should be attempted when feasible and clinically indicated.Filter retrieval is a patient-specific, clinically complex decision; On 17jul2018, per a report from computed tomography 2; ¿impressions: some of the filter struts have penetrated through the wall of the ivc into the pericaval/mesenteric fat.The filter is tilted posteriorly with its cone penetrating through the wall of the ivc into the perivaval fat.This may render the filter non removable.Addendum: the anterior right strut penetrates 7 mm through the ivc wall.The anterior left strut penetrates 7 mm through the ivc wall.The posterior right strut penetrates 4 mm through the ivc wall.The posterior left strut penetrates 0 mm through the ivc wall.¿.
