The reported event could be confirmed, although the device was not returned but images were provided for evaluation.A physical device inspection was not possible since the affected device was not returned and only the pictures were shared by the customer.Based on the images received, the nail holding bolt has deformed and worn out surface of the bolt could be observed. Some part of the nail holding screw is completely chipped off indicating use of intense force to remove the nail the holding screw.Based on the above investigation, the root cause of the failure is not related to deficiency of the device but is rather considered to be an off-label use by the user.Nail holding screws are not meant to be hit even for removal. As the device had been in use for a long time (manufactured in 2006), it can be safely said that it had fulfilled its tasks in former surgeries as intended.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of labelling did not indicate any abnormalities. The instruction for use states: ¿do not hit instruments unless they are specifically intended for impaction.¿ no indications of manufacturing or design related problems were found during the investigation. If the device is returned or if any additional information is provided, the investigation will be reassessed.
