Additional Manufacturer Narrative
Blank fields on this form indicate the information is unknown, unavailable, or unchanged.Investigation - evaluation a review of the documentation including the complaint history, device history record (dhr), manufacturing instructions (mi) and quality control of the device was conducted during the investigation. The complaint device was not returned; therefore, no physical examinations could be performed. However, a document-based investigation evaluation was performed.Cook has concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution.A review of the design history file found that the risks associated with this device are acceptable when weighed against the benefits.A review of the dhr for the complaint lot (9378479) and related wire guide sub-assembly lots revealed no related non-conformances.A database search found no other events associated with the provided complaint lot.As there are adequate inspection activities established, objective evidence that the dhr was fully executed, no related non-conformances, and no additional complaints from the same lot, it was concluded that there is no evidence that nonconforming product exists in house or in field.Additionally, there is no evidence to suggest that this product was manufactured out of specification.Cook also reviewed product labeling. No ifu (instructions for use) exists for the complaint device. However, there is a product warning label on the wire guide holder depicting a warning to not pull the distal spring coil portion of the wire guide through the needle tip.Based on the information provided, no product returned, and the results of our investigation, a root cause can be traced to component failure without a manufacturing or design defect. However, it is unknown if the wire was manipulated through the needle.Pulling the wire back to reposition can cause the wire guide to get caught on the sharp edge of the needle and can cause separation to the wire. Also, it is unknown if the patient¿s anatomy was tortuous. The appropriate personnel have been notified.Cook will continue to monitor for similar complaints.Per the quality engineering risk assessment no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
