Exemption number e2019001.Visual inspection was performed on the returned device. The reported failure to advance could not be replicated in a testing environment as it was based on operational circumstances.Device analysis noted that the hypotube was separated distal to the nose of the hub and the balloon had a radial rupture distal to the proximal balloon marker.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents/complaints from this lot.The investigation was unable to determine a conclusive cause for the reported failure to advance.A conclusive cause for the noted shaft separation and balloon ruptures could not be determined since limited information was reported. There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
