Event or Problem Description
Customer reported via phone call that reservoir was not able to lock in place when inserted into insulin pump.Customer¿s blood glucose was unknown.Customer stated that threading were messing up on reservoir compartment.Customer mentioned that sometimes reservoir will lock in place and sometimes will not lock.Customer stated that rubber piece fell off from the battery cap.Customer was advised to discontinue use of the pump and revert to backup plan.Insulin pump will be returned for analysis.
