The following were reviewed as part of this investigation: patient severity, trend analysis, applicable previous investigation(s), sample (if available), and applicable fmea documents.Based on a review of this information, the following was concluded: the complaint of foreign material on the infusion set needle was confirmed; however, the root cause was not identified. The product returned for evaluation was one photograph depicting a 22ga x 0.75¿ miniloc safety infusion set.The visible portion of the device included the needle housing, non-engaged safety mechanism and needle. No obvious usage residues were observed.Unidentified material was observed on the needle shaft, just proximal of the bevel. While unidentified material was evident in the submitted photograph, inspection of the photograph was insufficient to identify potential sources of the material.Consequently this complaint is confirmed as ¿cause unknown¿ at this time.Potential contributing factors include material deposition prior to device packaging and following device removal.A lot history review (lhr) of ascvs0065 showed no other similar product complaint(s) from this lot number.
