The following were reviewed as part of this investigation: patient severity, applicable previous investigation(s), sample analysis, applicable fmea documents, applicable manufacturing records, and labeling.Based on a review of this information, the following was concluded: the complaint of a connector detachment is confirmed and appears to be related to the use of the device. One 5 fr dl groshong catheter was returned for investigation.Residual material was present within the catheter and on the surface of the luer hubs.A paper note was returned with product information lot: recw0500 and ref: (b)(4).The sample was returned with the red luer connector slightly removed from the white over-sleeve.Both lumens were flushed with water using a 12 ml syringe and were patent to infusion. The samples were pressurized, and no leaks were observed. An attempt to remove the red connector from the white over-sleeve revealed it to require excessive force for it to be successfully removed.Microscopic observation of the proximal end of the extension tubing revealed no apparent damage.Based on the condition of the returned sample and force required to detach the red hub, it is likely that excessive force or manipulation contributed to the reported event; therefore, the complaint is confirmed.A lot history review (lhr) of recw0500 showed no other similar product complaint(s) from this lot number.
