Additional Manufacturer Narrative
The device history record was reviewed and indicated that the product was released accomplishing all quality standards. One decontaminated sample with lot number 1914405464 was received for evaluation. In addition, a photo was provided. After performing a visual inspection the reported issue was confirmed, the tubing was observed to have a cut tip.A gemba walk was performed by the multifunctional team (engineering, quality, production), and it was determined that the most probable root cause could be that the current fixture doesn't allow the operator to have proper visibility while doing the punching. As part of continuous improvements, a quality alert will be implemented and improvements will be made to the fixture, changing the current material to a light color. The process is running according to product specifications meeting quality acceptance criteria. We will continue to monitor the process for any adverse trends that require immediate attention.
