Additional Manufacturer Narrative
The associated device, used in treatment, was returned and evaluated.A visual inspection of the returned device could not confirm the stated failure mode. The device was manufactured in 2018 and exhibits signs of moderate wear / usage. The dimensional evaluation found the device to be within specification.A review of complaint history did not reveal additional complaints for the listed batch.A review of the manufacturing records did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident. At this time, we have no reason to suspect that the product failed to meet any product specifications at the time of manufacture. The medical investigation concluded that the complaint reports a ¿depth gauge for 1.8mm pegs/2.4mm screws¿ misaligned by 2mm.Due to this finding several screws had to be switched out for shorter screws. No clinical/medical documentation material was sent for this investigation. The depth gauge was returned for evaluation. The ¿failure mode is not confirmed, as part, functions and measures correctly and as intended.Measurements were taken at the 10, 20, 30mm marks. The impact to the patient was not reported except that the procedure was completed satisfactorily. The probable root cause for the depth discrepancy could have been caused by not locking sleeve on this part. The sleeve must be tightened down securely on to the small sleeve in order for an accurate measurement to be taken per the instructions.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.
