It was reported that the procedure was performed to treat a lesion in the left anterior descending artery.A 014 ht turntrac guide wire was advanced and an unspecified stent was deployed; however, the guide wire became jailed behind the stent. An attempt to remove the guide wire was made, but it unraveled and separated. The separated piece of the guide wire was trapped in the guide catheter with a balloon and removed as a unit. The procedure was successfully completed with a non-abbott guide wire. There were no adverse patient sequela and no clinically significant delay in the procedure. No additional information was provided.

G9: exemption number e2019001-permits numbering sequence to begin with 10000, to avoid duplication of report numbers due to process transition. There may be gaps in numbering for reports submitted during the transition period. The device was returned for analysis. The reported stretched coils and material separation was able to be confirmed. The reported entrapment of device was unable to be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that the guide wire became jailed behind the stent resulting in the reported entrapment of device.Manipulation of the device resulted in the reported/noted stretched and entangled coils and ultimately resulted in the reported/noted core and coil separations. As reported, the separated piece of the guide wire was trapped in the guide catheter with a balloon and removed as a unit.Additionally, some of the separated guide wire was not able to be removed as it was jailed behind the stent. There is no indication of a product quality issue with respect to manufacture, design or labeling.H6: patient code 2199 was removed.
