Event or Problem Description
Customer reported via phone call that the insulin pump had cosmetic damage and lip ring from reservoir came off.Customer's blood glucose level was unknown.Customer stated that the rubber on reservoir compartment came off.Customer reported that reservoir was unable to lock in place when inserted into insulin pump.Customer was advised to revert to backup plan.Insulin pump will be returned for analysis.

A test reservoir was installed and did not lock in place due to completely broken reservoir tube lip.Unable to perform the displacement test due to completely broken reservoir tube lip.Device had missing reservoir tube o-ring.
