According to the reporter, during laparoscopic inguinal hernia repair procedure, the device was difficult to articulate and when the surgeon attempted to fire the device on the first and second time no tack came out, and on the third attempt tacks release but fall into the patient cavity.To resolve the issue the surgeon retrieved the tacks from patient cavity and had to suture the patient instead in order to complete the case.
