Event Description
On (b)(6) 2019 information was reported that the patient is getting an mri.It is unknown if the mri is due a problem with the patient's device/therapy.Symptoms related to the patient's device/therapy were reported, but were not specified.The caller stated the patient reported that their ins has been dead for some time.Troubleshooting could not be done due to lack of access to product.No further complications were reported.No additional patient symptoms were reported.On 2019-nov-19 additional information was received from the patient.It was reported that the circumstance that led to the issues were that patient had been sick for over a year, weight loss, pains, unable to walk.Implant removed (b)(6) 2019.The cause of implant being dead was that implant was turned off for a month - would not reboot.Actions/interventions taken were that implant was reprogrammed, no relief in pain.The issues were not resolved.No further complications were reported.
