This report is being submitted as follow up no.1 to update section h3, and to provide the completed investigation results. One 9fr introducer sheath and dilator were returned for product evaluation.Visual inspection revealed that the sheath was broken close to the hub. There was a kink in the small part of the sheath shaft fragment that remained attached to the hub.The sheath shaft fragment that broke off from the hub was not deformed in any way, the sheath shaft was still round. The breakage was replicated using an ik sheath from the same product code and lot (rss902, xe03).The dilator was mated to the sheath and clamped at the distal end of the sheath.A knife was used to make a tiny cut in the sheath close to the clamp.Using a pair of pliers, the sheath hub was pulled upward, and the sheath broke almost immediately. The pattern of the breakage matches that of the sheath returned for evaluation by the customer. The sheath shaft also remained round and was not crushed or stretched.Based on the provided information and investigation results, there is no definitive evidence that this event was related to a device defect or malfunction.Based on the investigation results it is likely that a tear in the sheath shaft propagated due to aggressive forces during withdrawal of the sheath leading to the reported event. However, the exact cause of the reported event cannot be definitively determined based on the available information.
