Two 5.5mm twinfix ultra ti anchor assemblies, intended for use in treatment, were returned for evaluation. The devices were returned in the same shelf carton prohibiting lot verification, as a result the investigation will be added to complaints (b)(4).Visual assessment of each device confirmed the reported complaint of breakage. The distal tip of each inserter shaft has broken off at the weldment. Both anchors were returned and show significant damage to their threads. The break area of the shaft is splayed.Examination of the weldment showed adequate weld on each mating component. As reported the patient had hard bone and no prep devices were used to prepare the insertion site. The condition of the devices indicate they were subjected to excessive force during anchor insertion resulting in the reported breakage.Per the device ifu under precautions: use of excessive force during insertion can cause failure of the suture anchor or insertion device.A two-finger ao technique should be used to insert the anchor. The ifu also instruct the end user to drill in hard bone applications.A review of the manufacturing and complaint records was performed for the reported lot, there were no indications that would suggest that the device did not meet product specifications upon release into distribution.Further investigation is not warranted at this time.
