Additional Manufacturer Narrative

Product performance engineering reviewed the incident information; however, the product was not returned to abbott vascular for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history revealed no other similar incidents reported for this lot.The investigation determined the reported difficulty to remove appears to be related to operational circumstances of the procedure.Based on the reported information, it is likely that during advancement through the moderately calcified anatomy, the 014 ht command guide wire became kinked.Manipulation and/or inadvertent mishandling of the guide wire during the procedure, likely resulted in the core and polymer separation. The difficulty to remove was likely caused by the kinks noted on the guide wire. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
