Event or Problem Description
Information was received from a manufacturer representative regarding a trial patient who was using an external neurostimulator (ens). It was noted that the patient¿s trial started on (b)(6) 2019.It was reported that during the procedure, when attempting to place the second lead, it was determined that the tip of the lead was bent.It was noted that no environmental/external or patient factors were determined, as the lead appeared bent as soon as it was removed from its plastic sheath. They attempted to adjust the stylet within the lead without removing it completely, however, it did not change the significant bend in the lead.It appeared as if the lead tip was bent over on itself and that the stylet had pushed through the lead itself.A new lead was then opened and able to be successfully placed with no issues. There were no patient complications reported as a result of this event.

Additional Manufacturer Narrative
