=
> according to the information provided, it was reported that during use, the anchor head rolled making it impossible to continue screwing. The complaint device was received and evaluated.Visual inspection revealed a damage in internal portion of the handle. The suture was not returned and the anchor head was received separated from the device which indicates that it broke.Further investigation under magnification disclosed that the end of the metal shaft was bent and at the upper part of the anchor there were indications of a breakage. No other anomalies were noted. Therefore the complaint was confirmed.Anchor breakages are typically associated with off axis insertion, levering during insertion, hard bone quality or using incorrect instrumentation for preparing bone hole. This type of anchor breakage is consistent with historical investigations in which it was determined that the possible root cause was likely a combination of torque, bending axis insertion and bone hole.Other than this possibility, we cannot discern a root cause for this failure.A manufacturing record evaluation was performed for the finished device [4l69080] number, and no non-conformances were identified at this time, no corrective action is required, and no further action is warranted. However, depuy synthes mitek will continue to track any related complaints within this device family to monitor the extent to which this complaint is observed in the field.Device history lot
