It is alleged that the patient received a gunther tulip on (b)(6) 2009.It is alleged that the patient was injured without further explanation.Hospital and medical records have been requested but not yet provided.

Per a (b)(6) 2019 ct (computed tomography) abdomen: "findings: there is an ivc filter.The filter has a slight tilt of 11 degrees to the patient's left.The tip of the filter is 2 millimeters below the lowest renal vein.There is no evidence for filter migration.There is perforation of 4 struts of the filter into the surrounding mesentery.There is no aortic perforation, small bowel perforation, adrenal perforation, pancreatic perforation, renal vein or kidney perforation, lobar spine or psoas muscle perforation or perforation of the diaphragm.There is no filter struck fracture or bending.There is no vena cava stenosis.The apex of the filter is not touching the ivc wall".

Patient allegedly received an implant on (b)(6) 2009 due to deep vein thrombosis (dvt).Patient is alleging device tilt and vena cava perforation.Patient notes and further alleges experiencing "pain, suffering, loss of enjoyment of life, distress", and anxiety.
