Event Description
Information was received from a nurse regarding a patient who was currently receiving baclofen with concentration 2000 mcg/ml at a dose rate of 1065.6 mcg/day via an implantable pump for intractable spasticity and other spasticity.It was reported they a pump memory error occurred when the patient was in for a refill last week.The pump had not been alarming before or since and the patient was asymptomatic.At the time of the report it was reviewed that memory errors can occur due to interrupted or incomplete telemetry.The nurse confirmed that she was able to refill and update the pump without incident after seeing the message about the memory.It was noted that troubleshooting had resolved the reported issue.No further patient complications have been reported as a result of this event.
 
Event Description
Additional information was received from a healthcare provider (hcp) indicated the cause of the pump error was ¿operator error¿.The patient¿s weight was 300 pounds.No further complications were reported.
