Additional information: additional information received indicated that the patient had symptoms of constant blurry vision at all focal points.A refraction was performed to see if prescription lenses would resolve the constant blur. The lenses did not help resolve the symptoms of blur. The patient also had symptoms of extreme glare and halos preventing her from feeling safe to drive during the day or at night. The patient also had symptoms of dizziness. It was confirmed that all of these symptoms have resolved since the implant exchange. The patient is very happy with the outcome. It was also reported that the mechanical failure symptoms reported were first reported on december 10, 2019.The following fields was updated accordingly: section b3.Date of event: (b)(6) 2019.Section h6: additional patient code: 2227 - halo.Section h6: additional patient code: 2137 - blurred vision.Section h6: additional patient code: 3191 - glare.Section d10.Device available for evaluation? yes; returned to manufacturer on: 1/30/2020 section h3.Device returned to manufacturer? yes.Device evaluation: the sample was returned in a lens case at the manufacturing site for evaluation. The product was inspected using a microscope with a 12x magnification. It can be seen that the lens is contaminated, dust particles are present. This is expected as the lens was transported from controlled environment during manufacturing to a non-sterile, non-environmental controlled area.Investigation of the return sample and the condition of the return sample do not suggest the dust particles are introduced during manufacturing. The complaint cannot be confirmed.Manufacturing record review: the manufacturing records for the product were reviewed. The product was manufactured and released according to specification.A search in the complaint system revealed that no similar complaints were received for this production order number. Conclusion: as a result of the investigation, there is no indication of a product quality deficiency. All pertinent information available to johnson & johnson surgical vision, inc.Has been submitted.
