Event or Problem Description
Physician intended to use a silverhawk atherectomy device and nitrex guidewire during treatment of a calcified lesion in the patient¿s proximal right superficial femoral artery (sfa)/tibial artery of diameter 6mm.Slight vessel tortuosity and severe calcification is reported.Ifu was followed and the device was prepped without issue.Lesion located in the patient¿s tibial vessel.Treatment with the atherectomy device was successful.Resistance was noted when attempting to remove the device.Fluoroscopic visualization of the device showed that the nitrex wire was looped in the iliac artery. When the device was pulled with more force, the tip broke free, but remained on the wire. When an attempt to snare failed, the operator decided to surgically remove the device and perform an endarterectomy of the cfa and proximal sfa.No parts of the device remained in the patient, and no further incident was noted.

Additional Manufacturer Narrative
Additional information: the attempt to snare failed as the angle needed to steer the wire down to get the snare in place was too acute. The wire was left in place and removed during endarterectomy. No further patient injury was reported for this event. If information is provided in the future, a supplemental report will be issued.

Additional Manufacturer Narrative
Additional information: the snare was intended to grasp the wire and/or device tip and retrieve it from the body of the patient. There was a soft bend in the patient's sfa.The attempt to navigate the snare into position was causing the tip of the silverhawk to migrate into the sfa.The surgeon had made the decision to abort percutaneous retrieval attempts and proceed to surgical removal of the wire and the atherectomy device tip.If information is provided in the future, a supplemental report will be issued.

Additional Manufacturer Narrative
Image review: two cine images were returned for reviewed.A silverhawk could not be identified in either image provided. The suspected target area could not be identified. On cine image shows a guidewire within the vessel and a possible marker band from a device, but it could not be identified as to what device the marker band may have belonged. If information is provided in the future, a supplemental report will be issued.
