It is alleged that the patient received a gunther tulip on (b)(6) 2005.It is alleged that the patient was injured without further explanation.
 
Event Description
Patient allegedly received an implant on (b)(6) 2005, via the right internal jugular vein due to pulmonary embolism (pe) and deep vein thrombosis (dvt).Patient is alleging vena cava perforation and tilt.The patient further alleges trouble sleeping due to stress and anxiety of filter moving further and having more side effects.(b)(6) 2018, per a report computed tomography (ct); ¿positive for caval perforation.Superior extent of ivc filter superior l2 vertebral body.Inferior extent mid l3 vertebral body.A total of 4 prongs have perforation the ivc.Maximum distance prongs perforated 4mm.Coronal images 5 degree tilt left to right.Sagittal images 3 degree tilt posterior anterior series.Diameter of ivc directly above filter 28mm x 20mm.¿.
