Additional Manufacturer Narrative
H.6.Investigation summary two photos were received and evaluated. One photo depicted a single package from the top web view, confirming batch #9170943 (p/n 305921).One photo depicted a safetyglide needle attached to a small nonbd syringe with a luer-lok adapter, all held by gloved hands. The safety shield was observed broken at the hinge with the tip portion held separate from the rest of the needle in one hand.Furthermore, a device history record review showed no rejected inspections or quality issues during the production of the provided lot number that could have contributed to the reported defect.Possible root cause is associated with the safetyglide assembly line. The plastic hub is placed under the cannulator then the needle is positioned and assembled to the plastic hub adding the epoxy to fix it; after that, a plastic safety mechanism is assembled, then at the end the plastic shield is assembled to the part.In this case, the safety mechanism was not properly assembled. No corrective actions are necessary based on the defective rate identified.Batch 9170943 is considered in compliance with our product specification requirements.Complaints received for this device and reported condition will continue to be tracked and trended.Information will be captured on trend reports and monitored monthly. Our business team regularly reviews the collected data for identification of emerging trends.
