Additional Manufacturer Narrative
Block h6: device code 1069 captures the reportable event of guide catheter broken.Block h10: the returned flexima biliary stent was analyzed, and a visual evluation noted that the stent was found kinked in the guide catheter at the distal section. The push catheter suture hole was observed torn. However, not bent/kink were observed in the stent, also, the guide catheter was not found broken. The suture was not returned for analysis. The reported event was not confirmed.Based on the product analysis, the issue occured during the procedure, it is most likely that during the procedure the device could have been manipulated, handling and manipulation during its use could contribute to the found kinks in the device. The kinks in the guide catheter could create resistance and/or friction in the device use.The continuous attempts to release the stent in addition to the encountered kink could result in the push catheter suture hole torn. This device condition could have generated that the suture loose, once that the suture is loosed could cause the stent premature deployment reported issue. Therefore, adverse event related to procedure, is selected as the most probable root cause for the complaint.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A search of the complaint database confirmed that no similar complaints exist for the specified lot.A labeling review was performed and, from the information available, this device was used per the directions for use (dfu) / product label.
