Event Description
Information was received from the healthcare provider (hcp) via a device manufacturer representative regarding a patient receiving gablofen (2000 mcg/l at 350mcg/day) via an implantable infusion pump.It was reported that the patient had a return in spasticity.A dye study was performed and showed that the drug was not getting to the catheter tip but the point of occlusion was unknown.During surgery a leak caused by a hole was discovered between the spinal and pump segment; a new collet was used to reconnect segments after removing the broken catheter segment.It was noted that the spinal segment of catheter was left in and that pump segment was removed discarded of.The issue was resolved and the patient was alive with no injury.No further complications have been reported as a result of this event.
