Event or Problem Description
Medtronic received a report that the axium coil experienced resistance, became stretched, and failed to detach. The patient was undergoing surgery for treatment of ccf.It was noted the patient's blood flow was normal and vessel tortuosity was moderate. It was reported that after deploying a few axium coils successfully, the healthcare provider (hcp) prepared the fourth axium coil per the ifu.It had been backflushed by introducing it into the rhv, and then into the microcatheter. The coil was repositioned for the second time, and then the hcp found resistance. The coil was stretched and was not getting detached. As a result, the coil was removed and replaced by another product to complete the procedure. It was indicated that all devices were prepared as per the instructions for use (ifu), and no patient symptoms or further complications were reported as a result of this event.Ancillary devices include a chikai black guidewire, echelon microcatheter, neuron guide catheter, cook shuttle sheath.
