Event Description
It was reported that the patient had a left ventricular assist device (lvad) placed, during which the right atrial (ra), left ventricular (lv) and right ventricular (rv) leads dislodged.The physician decided to replace the rv lead and plug the ra and lv lead ports.The ra and lv leads were explanted.No patient complications have been reported as a result of this event.

Product event summary: the full lead was returned, analyzed and no anomalies were found.Visual analysis of the lead indicated apparent explant damage.If information is provided in the future, a supplemental report will be issued.

(b)(6) 2020: it was further reported that the rv lead had perforated the right ventricle and dislodged to the right ventricular outflow tract.

It was further reported the patient presented with severe nausea, bloating and cardiogenic shock (low cardiac index and high pulmonary capillary wedge pressure (pcwp)).The patient was treated per swan guided therapy with iv lasix and was implanted with a bridge to transplant medical device.
