Event or Problem Description
Customer reported via phone call that they experienced high blood glucose level of over 400 mg/dl.Troubleshooting was performed and found a bend in the set cannula.The customer stated insulin pump had insulin flow blocked alarm.The insulin pump was not returned for analysis.
