It was reported while a 24- hour tacrolimus infusion was running, the tubing set ballooned at the pumping segment.Furthermore, it was noted recurrent occlusion alarms and visually checked for kinks in the line and restarted infusion.The device provided occlusion alarm once again, paused the infusion, removed the tubing, and then manually flushed the set through the needleless valve closest to the patient.It was noted that the phaseal optima clamp at central venous catheter was intact but the injector/connector segment was slightly coming apart.Reinserted injector/connector and reclosed the clamp.The tubing set was reinserted and that is when the balloon in the pump segment was noticed.The event occurred at the cancer center.There was not adverse effects caused to the patient.
