Event Description
It was reported that two (2) effluent bags leaked from ¿the seam around the medication port.¿ this was observed during use of peritoneal dialysis therapy.There was no patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
Correction: device codes: add component code 432 (remove 495 reported on initial).Additional information: two (2) devices were received for evaluation; however, an evaluation was not performed as the reported leak is a known issue with this lot.The cause of the condition was determined to be related to the manufacturing process involved in creating the welds of the effluent bags.To address this issue, a worn piece of manufacturing equipment was replaced and the manufacturing process has been updated to perform 100% pressure testing on all bags.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
