Event Description
Information was received from a healthcare professional (hcp) via a company representative (rep) regarding a patient who was receiving morphine with a concentration and dose of 10 mcg/ml at 0.5 mg/day via an implantable drug delivery pump for non-malignant pain.It was reported that the hcp was doing a refill when they aspirated 18 cc of drug out of the pump.It was unknown if there were any environmental/external/patient factors.The refill was done but more drug was pulled out than initially expected.The hcp watched the volumes move forward as a troubleshooting action.The issue was not resolved.No patient symptoms or further complications were reported.

Additional information was received from a healthcare provider (hcp) via a company representative (rep) indicated the (b)(6) 2019 refill notes showed the recorded reservoir volume was 4.1 and the aspirated volume was 10 mls.It was noted ¿today¿ the recorded residual volume was 2.4 mls and they aspirated 18 mls and this was ¿concerning.¿ the cause of the volume discrepancies was unknown and no actions/interventions were taken yet.It was noted they would do a catheter access port (cap) study after the covid crisis.The patient¿s weight at the time of the event was unknown.The patient was not experiencing any symptoms and the volume discrepancy was not resolved.The pump was currently still implanted.No further complications were reported.

On 2020-june-22, additional information was received from an healthcare professional (hcp), via a manufacturer representative (rep).It was reported the issue was "higher than expected volumes".The environmental, external, or patient factors that may have led or contributed to the issue was "unknown".Diagnostics/troubleshooting reported "catheter study tried, but couldn't be done because pump is flipped".Interventions/actions taken was 'surgery being booked".The issue was not resolved at the time of this report.The patient's status was alive - no injury.

Additional information was received from the manufacturer's representative (rep) on (b)(6) 2020.It was reported that the cause of t he flipped pump was unknown and it was unknown if the pump had affected any other devices in the system.The cause of the volume discrepancies was also unknown.The patient's surgery was scheduled for 2020-jul-02.It was unknown if any of the patient's devices would be returned for analysis.

Additional information was received from a healthcare provider via a company representative.The patient was brought to the operating room on (b)(6) 2020.A catheter revision was performed.The new starting dose was a minimum rate as per the physician.The pump was moved to patient¿s side per physician to avoid belly fat and excess skin to prevent the pump from flipping in the future.

Event Description
Additional information was received from a rep.It was reported that the reason for the catheter revision was because the patient kept flipping the pump in their abdomen and the catheter was all twisted.A pocket revision was also performed and the pump was moved to the patient's side.It was reported that the old catheter from the anchor to the pump was removed and a new catheter was implanted.It was reported that the cause of the pump flipping wasn't determined but the rep was showed the patient flipping it themself.The rep was not aware of the volume discrepancy and asked if the twisted catheter could have been the cause.It was reported that the patient's pump was restarted at min rate and would titrate back up.It was reported that the explanted catheter was discarded and would not be sent back.
