Event or Problem Description
Information received by medtronic indicated that on the rewind process, the piston was not going all the way down and reservoir was not completely fit in to the compartment.Customer did self test and it was passed.No harm requiring medical intervention was reported.The insulin pump will be returned for analysis.
Additional Manufacturer Narrative
Device received with completely broken reservoir tube lip and missing o-ring (reservoir tube).Unable to perform all functional testing including the displacement, rewind, basic occlusion, occlusion, prime/a33 and excessive no delivery test due to completely broken reservoir tube lip.
