Additional Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that during advancement resistance was met with the severely calcified anatomy resulting in the reported difficult to advance.During pull back to confirm the position to implant, interaction with the severely calcified anatomy resulted in the reported difficult to remove.Manipulation of the device ultimately resulted in the reported stent dislodgement. There is no indication of a product quality issue with respect to manufacture, design or labeling.

Event or Problem Description
It was reported that the procedure was to treat a concentric de novo lesion located in the proximal left anterior descending coronary artery that was severely calcified.Following pre-dilatation with a 3.0 mm balloon, a xience sierra stent delivery system (sds) was advanced to the lesion against slight resistance. When the sds was pulled back to confirm the position to implant, due to the calcification in the lesion, the stent dislodged. The dislodged stent was retrieved with a snare. There was no adverse patient sequela or a clinically significant delay in procedure.Another xience sierra was successfully implanted. No additional information was provided.
