This report is being submitted as follow up no.1 to update section h3, and to provide the completed investigation results. One 90 cm 6fr destination sheath and dilator were received for evaluation.Visual inspection revealed that both components were returned folded in a u-shape.The sheath was viewed under microscope and it was noted that the sheath was flattened 45 cm from the hub.No other anomalies were noted on the dilator. The packaging for the device was not returned for evaluation.Based on the provided information and investigation results, there is no definitive evidence that this event was related to a device defect or malfunction. It is likely that mishandling of the packaging tray may have caused the damage noted on the sheath. However, without the return of the packaging tray, the exact cause of the reported event cannot be definitively determined based on the available information.
