The device was not returned for evaluation.A review of the lot history record and complaint history of the reported lot could not be conducted because the lot number was not provided.The investigation was unable to determine a conclusive cause for the reported deflation issue; however, the reported difficulty removing the device appears to be related to circumstances of the procedure.There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
 
Event Description
It was reported that the procedure was to treat a 90% stenosed lesion in the left anterior descending artery.The lesion was predilated with a 4.50x12mm nc trek balloon dilatation catheter (bdc).The balloon was inflated twice to 15 and 18 atmospheres.During deflation negative was held for two seconds and resistance was met with anatomy during removal.After several attempts the balloon finally withdrew successfully with the guiding catheter.It was observed the bdc remained partially inflated.Another 4.50x12mm nc trek bdc was used to successfully complete the procedure.There were no adverse patient effects and no reported clinically significant delay in the procedure.No additional information was provided.
